Prepare for Your Fitting:

If we are filing your insurance, we must have a physician's order.  We can not provide your product without it as your insurance company requires this.

Please bring:

• Insurance Card(s)
• Physician's Order
• Photo Identification

For breast care fittings:  Your favorite bra to give us an idea of your expectations

For wig fittings:  a picture of your vision for your style

We accept insurance assignment up to the current allowable rates and limitations.  Any deductibles, coinsurance, and upgrade charges are your responsibitliy.  Please understand your personal insurance policy.  The most commonly used insurance codes are:
